Bristol Hospice

Bristol Hospice provides comfort, dignity, and quality of life for patients facing a terminal illness — wherever they call home, whether that's a private residence, an assisted living community, or a nursing home.

An interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers manages pain and symptoms, provides medications, equipment, and supplies related to the hospice diagnosis, and offers emotional and spiritual support for the whole family, including bereavement support after a loss.

Bristol is Medicare-certified and holds a 99.5% HIS score — above the 91.5% national Medicare average — with a 4.7-star Google rating from families nationwide across nearly two decades of care.

Matt Pokorny, DPT, ATC, is the hospice liaison for the Denver area. He helps families and referral partners understand eligibility, timing, and next steps, with customer service and RN support available by phone.
